DETAILS

Housing Stabilization Services (HSS)

Housing Stabilization Services, offered by Friendship Care LLC, is a Medical Assistance benefit designed to assist individuals with disabilities, seniors, and those at risk of homelessness in finding and maintaining housing. Recognized as state plan Home and Community-Based Services (HCBS), HSS comprises three distinct services: Housing Consultation, Housing Transition, and Sustaining.

Eligibility for HSS is open to individuals facing various living situations, making it a suitable option for those already residing in unsuitable conditions, experiencing homelessness, living with family, or in institutional settings like hospitals, nursing homes, or intermediate care facilities (ICF).

The program emphasizes creating a housing-focused person-centered plan, tailored to individual needs, preferences, and informed choices.

To commence the process, we engage in comprehensive planning, beginning with an assessment. Our team works collaboratively with you to identify suitable housing, facilitate the move, and conduct follow-ups to ensure a smooth transition.

To initiate the housing process, our Housing Consultant Specialist will meet with you to complete a housing-focused person-centered plan, which guides us in providing the necessary support for finding and maintaining your housing.

Housing Consultation is a service aimed at developing a person-centered plan for individuals without MA case management services. This service assists in building a housing-focused person-centered plan, supporting individuals in making informed choices regarding housing transitions or sustaining providers. Additionally, consultation services connect individuals to non-housing-related resources identified in their person-centered plans.

Housing Transition is a service dedicated to supporting individuals in finding suitable housing, while Housing Sustaining focuses on helping individuals maintain their housing.

To get Housing Stabilization Services, you must:

  • Have Medical Assistance (MA) coverage

  • Be 18 or older

  • Have a disability

    • Your disability does not have to meet Social Security’s adult standards for disability. Disabilities can include physical disabilities, mental illness, substance use disorder, and other conditions.

      Be in one of the following situations:

    • Experiencing homelessness

    • At risk of homelessness (including being doubled up, needing services to keep your housing, or you experienced homelessness in the past and now are being discharged from a correctional, medical, mental health, or substance use disorder treatment center without a permanent place to live)

    • Moving out of (or moved out of in the last six months) an institution, nursing facility, or certain other group settings like Board and Lodge or Adult Foster Care, or

    • At risk of institutionalization.

      Be assessed to need help with at least one of these disability-related areas:

    • Communication

    • Mobility

    • Decision-making, or

    • Managing moods or behaviors
